1. Print the form on the link below for the paper application to order a Florida Medical Marijuana Card via mail. Most cards are ordered electronically from the link emailed to patients.

2. Indicate on the third page whether it is an initial, renewal, or minor application.
3. Complete all required fields and include:

- A passport-style photo that meets the guidelines. This can typically be obtained at Walgreen's, AAA Motorclub, Walmart, etc.
- A copy of Florida Driver's License or the required proof of residency for seasonal residents
- A $75.00 check or money order made out to the Florida Department of Health with your Patient ID Number included on the memo line
